How To Register

Call in at the surgery and ask the receptionist for a GMS1 registration form and a new patient registration pack. The receptionist will hand you a pack which includes a health questionnaire, a leaflet explaining all about the Surgery, information on the new Accessible Information Standard, information on Summary Care Records and Carers, Patient Online Access and also our complaints policy and complaint form (even though we hope that you never need to use it!).   
 
The questionnaire and the GMS1 need to be handed back to the receptionist duly completed before registration can take place.  We require all patients to have an appointment with the doctor or the Practice Nurse just to go through your medical history.
 
We have a defined practice area within the Parish of Ashover and 400 metres outside the boundary.   A map showing our boundary is on the wall in the Surgery. Patients leaving the practice area will be asked to register with a practice in their new locality.
 
Please note that if you live within 1.6km of a pharmacy, the Practice will be delighted to accept you as a patient, but due to NHS Dispensing Regulations, we are unable to dispense to you.  We are also unable to dispense to you if you move to the area from another Health and Wellbeing Board.  If you require further clarification on this, please speak to a Dispenser or ask to speak with the Practice Manager.
